About the VNA

History

Southcoast Visiting Nurse Association is the oldest not-for-profit home health and hospice care agency serving communities throughout Southeastern Massachusetts and East Bay, Rhode Island. Originally founded in Taunton, Massachusetts in 1905, the agency was created in response to high infant mortality rates and the spread of infectious diseases in local cities and towns. Today, we offer specialized home health and hospice care services utilizing sophisticated medical technology and a highly skilled and dedicated team of health care professionals.


 

Mission

Our mission is to provide high-quality, comprehensive health and hospice services in the home setting and community-based programs which foster improvement and maintenance of health, prevention of disease and disability, and compassionate end-of-life care. We are committed to providing the best home health and hospice care services possible in all the communities we serve, without regard to an individual’s insurance status or ability to pay for services.

Programs and Services

Southcoast VNA offers the most complete and comprehensive range of quality home health and hospice services. We help individuals recover or live with illness in the comfort and privacy of home including individuals who have had surgery, are chronically ill, have a disability, have terminal or life-threatening illness, or need some help to complete everyday personal care activities.

Our professional staff provide compassionate care specially tailored to each individual's needs and are available 24 hours a day, 7 days a week and include:

Skilled Nursing

VNA nurses are either registered or licensed professionals and provide medical care and education for helping patients recover from illness or injury at home. Nurses coordinate plans for each patient’s medical needs and are able to provide a wide range of clinical services in the home including supervision of medical regimens and changing dressings. They are also able to provide cardiac, respiratory, diabetes, maternal-child health, and pain management programs.

Rehabilitative Therapies

Staff includes licensed physical and occupational therapists and speech-language pathologists. Physical therapists provide a variety of services including motor development skills for children and pain treatment for adults. Occupational therapists help patients achieve greater independence and help improve ability to perform every day activities. Speech/language therapists help patients having difficulty with speaking and/or swallowing. Medical Social Work assesses the social and emotional circumstances of patients and develops plans for increased support at home. Other specific services include crisis intervention, grief work therapy, nursing home placement assistance and counseling for financial problems.

Hospice and Palliative Care

 


Hospice & Palliative Care Program provides compassionate care and support at home for individuals and their families who have a terminal or life-threatneing illness.

Chronic Disease Management

Uses daily, in-home telehealth monitoring along with home nursing care to improve management and maintenance of chronic disease for patients with serious health conditions such as heart failure, chronic pulmonary disease and diabetes.

Infusion Therapy

Infusion allows patients a smooth transition from the hospital setting to continue their IV therapy treatment in the comfort of their own home or alternative setting. Patients can continue their infusion or nutritional therapies under the safe and effective plan implemented by a clinical specialty team.

Dietician/Nutrition Services

A registered dietician assists patients in meeting their nutritional needs and help patients better understand special diets prescribed by their physician.

Home Health Aides

Certified nursing assistants provide patients with daily personal care assistance such as bathing, grooming, meal preparation and exercise.

Specialty Services and Programs

Southcoast VNA also offers a wealth of specialty services designed to provide the best managed care for patients. These include a dedicated wound team, in-home coagulation monitoring, respiratory therapy, phlebotomy services, personal monitoring systems in partnership Philips Lifeline®, pediatrics, home safety assessments and fall prevention programs. In addition, we offer a variety of wellness clinics and educational programs as a benefit to the communities we serve.
