You Have The Right To Choose Your Home Health Care or Hospice Provider

If your doctor decides you need home health or hospice care, you have the right to choose which agency will provide you with the care and services you need. Your choice should be respected by your doctor, hospital discharge planner, or other referring agency. Although some hospitals have their own home health agency, you do not have to choose the hospital agency. You have the right to choose any agency that you believe will meet your medical needs.

How To Choose the VNA?

You have the power to choose your own health care provider. You can even let your doctor or hospital discharge planner know in advance that you want the VNA for your home health care needs.

To help you be prepared, the VNA is able to provide you with a free "I choose the VNA" wallet card. Keep this card with you and present it to your doctor or other health care provider who begins to discuss home health or hospice services with you. Presenting this card demonstrates that you have already chosen the VNA for your home health care.
