Our Admissions Department is staffed by registered nurses who process referrals from physicians, hospitals, skilled nursing homes and community agencies. The VNA coordinates all aspects of home and hospice care and works closely with the patient's physician to ensure that appropriate services are in place. To make a referral, VNA needs the following information: patient's name, address, telephone number and birth date; physician's name; diagnosis; and form of health insurance coverage.

• The Admissions Department is also able to connect the patient and family with other community resources, such as elder services, Meals-on-Wheels, and pharmacy assistance programs.
• We facilitate the ordering and delivery of medical equipment and supplies.
• Payment is accepted from private insurance, Medicare and Medicaid, as well as direct patient payments.
• Southcoast VNA provides care to anyone who needs it, regardless of insurance status or ability to pay for service. Financial assistance is available for qualified applicants.
• Our Admissions Department also has nurses who are reimbursement specialists and can help with questions about your home care benefits.
